How to Avail of SSS Maternity Benefit

When a woman gets pregnant, it is one of the happiest moments in her life, but both parents need a certain amount of money to afford all the medical and hospital expenses.

SSS give benefits not just for retirement insurance, but also maternity benefit to female SSS members.

What are some of the requirement needed to avail the SSS benefits?

To receive the benefits from SSS, you must submit maternity notification ahead of time.

For employers and employees:

SSS Maternity Notification Form needs to be accomplished to receive the benefits of pregnant woman. To make the form valid, the member who is pregnant must immediately notify first her employer and tell about the probable date of her childbirth, and must do this 60 days before passing the form. Aside from the Maternity Notification Form, the member must submit proof of pregnancy.

After the receipt of the notification given to the employee member, the employer should immediately notify the SSS through the maternity notification form and proof of pregnancy that was given to him.

Look for the SSS branch counter to give the notification to settle it offline or you choose to send the notification through online transaction by sending it to SSS website (www.sss.gov.ph). To make this procedure, the member/employer should be a registered user.

For unemployed/self-employed and voluntary members:

The benefits are not only limited to employed citizen, but also for unemployed or self-employed and those who voluntary paying members of SSS.

Unemployed or self-employed and voluntary members also need to notify the SSS directly.

Take note: It is important for employee to notify the employer, or the SSS, same with the unemployed, self-employed, and voluntary paying member. If both parties failed to notify the SSS, the application may be denied.

What are the requirements?

For notification process

For Employee, Unemployed, Self-Employed, and Voluntary members:

  • Maternity Notification Form
  • UMID/SSS biometrics ID card/two (2) other valid IDs, both need a signature, at least one (1) with photo and date of birth
  • Proof of pregnancy (ultrasound report)

For reimbursement process

  1. Employed members
  • Maternity Notification Form duly stamped and received by SSS
  • Maternity Reimbursement Form
  • UMID/SSS biometrics ID card/two (2) other valid IDs, both need a signature, at least one (1) with photo and date of birth

The member’s SSS digitized ID or E-6 acknowledgement stub must be presented together with the member’s two (2) valid IDs. This procedure should be done by the authorized company representative. 

This will ensure receipt of benefits by members. The company representative’s presentation with their very own SSS digitized ID and the blue card will be part of this part of this requirement.

  1. For members who are separated
  • Maternity Notification Form duly stamped and received by SSS
  • Maternity Reimbursement Form
  • UMID/SSS biometrics ID card/two (2) other valid IDs, both need a signature, at least one (1) with photo and date of birth
  1. For those who are self-employed and voluntary members
  • Maternity Notification Form duly stamped and received by SSS
  • Maternity Reimbursement Form
  • UMID/SSS biometrics ID card/two (2) other valid IDs, both need a signature, at least one (1) with photo and date of birth

Aside from these requirement, there are still need of additional requirements for certain circumstances.

Other requirements:

  1. When the delivery is normalregistered birth certificate is needed to certify the type of delivery. If in case the child died even before was born, or the child is stillborn, the requirements are: duly registered death and fetal death certificate.
  2. Caesarian delivery – present the copy of operating room record/surgical memorandum to prove and certify the operation.
  3. When miscarriage and abortion happens – present the proof from the attending physician and dilatation which is the obstetrical history stating the number of pregnancy and curettage (D&C) report for incomplete abortion, also present the hystopath report complete abortion, the age of gestation, and the pregnancy test before and after the procedure.

Look for the SSS office near you where your record are based, so the requirements should be valid.

Of course, as how the title itself define the main prospect of this benefit – no other than the female members. The over-all qualifications to gain the benefits from SSS maternity are:

  1. Within the 12-month period, the female member should pay her three (3) monthly contributions preceding the semester of her childbirth or miscarriage.
  2. The female member must have succeeded to notify the SSS through her employer if she is employed, or if she belonged to the group of unemployed or self-employed, she must headed the SSS directly to notify.

Starting on May 24, 1997 when Social Security Act of 1997 took effect where the maternity benefit will be paid only for the first four deliveries or miscarriages.
