How to Get a PhilHealth ID

Members must have provided their Member Data Record (MDR) or PhilHealth ID Before claiming any PhilHealth benefits. Most of the time, PhilHealth members overlooked the ID even though it’s free.

Is it worth it to have a PhilHealth ID? Yes, it’s a valid government ID.

You can use it for pre-employment requirements or commercial services requirements that require proof of identity like banks, money transfers, telecoms etc…

Make sure to be a PhilHealth Member first before applying for PhilHealth ID.

How to apply as PhilHealth member:

For employed individuals, your HR department will process your PhilHealth membership for you. Also, they will pay your monthly contributions which will be deducted from your salary.

For unemployed or self-employed individuals, you can register as “Voluntary Member” and pay your monthly contributions personally. Paying your PhilHealth contributions can also be done monthly, quarterly, semi-annually, or annually.

What are the requirements for PhilHealth membership?

Go to your nearest PhilHealth Local Insurance Office (LHIO) and submit the following requirements.

  • Two accomplished PhilHealth Member Registration Form (PMRF), you can download it here.
  • Two 1×1 ID pictures (white background)
  • Two valid IDs or a photocopy of your Birth Certificate (NSO certified)

Once done processing your requirements, you will receive your PhilHealth number which you will use when paying your monthly contributions.

How to get PhilHealth ID and PhilHealth Insurance ID Card:

There are two PhilHealth IDs that you can apply.

Regular PhilHealth ID – an old school paper like ID that you can get for free at any PhilHealth offices. Just provide your PhilHealth number and they can print your ID right away.

Note: This ID doesn’t have a picture yet, you can put a 1×1 ID picture on it if you want.

PhilHealth Insurance ID Card – a modern digitized ID issued to PhilHealth members in all sectors in the Philippines. Members can get one for ₱90 at any PhilHealth LHIO.

 

For dependent ID card, you must pay ₱358 to any Bayad Center outlet or ₱360 at any SM mall payment center. Also, you need to present at least two (2) valid IDs of your dependent.

  • Discount to all hospitals in the Philippines.
  • 20% to 80% discount on flu vaccines on PQ Health Shield and GSK
  • 20% discount on regular items in Vivian Sarabia Optical
  • Free eye exam from Vivian Sarabia Optical
  • 15% discount on generic drugs on Watson’s, Rose Pharmacy, South Star Drug, and The Generics Pharmacy
  • 10% discount on drug testing services at JNW Drug Testing sites

Having a PhilHealth ID is an effective way to claim benefits in both private and public health care institutions. Make sure to always update your PhilHealth membership and secure your ID for medical emergencies.
